DOCUMENTS THAT HAS TO BE PRESENTED DURING THE INTERVIEW FOR A U.S. VISA:
1. Bank statement for the last three (3) months and both current and former bank account passbooks
2. Employment certification including salary, tenure and position
3. Form W-2
4. Income tax return with Bureau of Internal Revenue (BIR) or bank stamp
5. Audited financial statement with BIR or bank stamp
6. Pay slips for the last three (3) months
7. Credit card statements for the prior three months
8. Vehicle registration with official receipt
9. Land titles (no certified copies please)
10. Pictures of famil, home or business
11. Wedding photos
12. Marriage certificate printed on the Philippine National Statistics Office security paper, if applicable
13. Birth certificate printed on the Philippine National Statistics Office security paper
14. For students, certificate of school registration
15. Certification of membership to legitimate organization(s)

REQUIREMENTS FOR RENEWAL OF PASSPORT:
1. Personal appearance of the applicant shall not be required and the application may be filed by:
a.) Any agency duly acccredited with the Department
b.) By any the following members of the immediate family
of the applicant authorized in writing by the latter
stating the relation to the applicant:
* Applicant's spouse
* Applicant's parents/children
* Applicant's brothers/sisters
* Applicant's legal guardian/guardian having custody
of the child
c.) A person authorized in writing by the applicant
provided the release of passport is only to applicant.
(Personal appearance may be required as warranted by
circumstances).
2. If passport being renewed is brown or issued prior to 01 May 1995
a.) Old passport and photocopy of passport pages 1,2,3
(amendment) and the pages showing latest Bureau of
Immigration departure and arrival stamps.
b.) Any documents with complete middle name (Birth
Certificate, Baptismal Certificate, Driver's
license, Company I.D., Voter's I.D., etc.)
3. In case passport being renewed is green or issued after 01 May 1995:
a.) Present old passport and submit photocopy of passport
pages 1,2,3 (amendment) and the pages showing latest
Bureau of Immigration departure and arrival stamps.
4. For married women who chose to adopt surname of husband,
this must be submitted:
a.) Certified true Copy and photocopy of marriage Contract
issued by the NSO must be submitted
5. For minors (below 18 years old):
a.) Personal appearance of either parent before the
passport issuing office.
b.) If minor is not traveling with either parent, an
original and photocopy of DSWD Clearance and Affidavit
of Support and Consent must also bre submitted, If
both parents are abroad, such an affidavit must be
authenticated by the nearest Philippine Embassy or
Consulate.
c.) If both parents are abroad, person applying in behalf
of the minor must submit a Special Power of Attorney
(SPA) authenticated by the nearest Philippine Embassy
or Consulate
d.) Passport and photocopy of the passport of person
traveling with the minor.
STEPS IN APPLYING BIRTH, MARRIAGE, DEATH & CENOMAR AT CENSUS-SERBILIS CENTER:
1. Requester accomplishes application form (AF) at the center and proceeds to the Screener for AF date/information check
2. Requester submits AF and pays corresponding amount to Collecting Officer (CO). CO generates OR then instructs requester to proceed to releasing area.
3. NSO processes the request.
4. Upon notification of the Receiving Clerk, requester presents OR and claims the result.
WHAT DOES SAME DAY VOIDING MEAN?
- Tickets issued during the day should be cancelled in the
computer before the end of the day.
Failure to do so: Ticket will be submitted for refund
with corresponding refund charges ranging from $20-$50.
